We are a specialized healthcare facility that serves as a bridge between an acute care hospital and home, providing a lower-intensity, cost-effective level of care for patients recovering from illness, injury, or surgery. Our facility offer medical services, rehabilitation therapies, and ongoing support for patients who need more intensive care than a nursing home provides but do not require the high-intensity services of a traditional hospital. The primary goal is to help patients regain independence and prepare for a successful transition back to their homes.
